BASIC RULES AND REGULATIONS
FOR PHYSICAL EDUCATION
2003-2004
Dear
Middle School Student,
Welcome to physical education! We are looking forward to an exciting
time with you here. Our goal in physical education is to provide you with
the skills and knowledge to enable you to become a lifelong physically
active person. We will provide you with a variety of activities to get you
moving and hope that you have fun!
The following guidelines have been written in order to help you
succeed in and enjoy your class. Read
them carefully; discuss them with your parents(s) or guardian(s).
We look forward to helping you reach your potential.

I. Physical
Education Attire
A. The following items are required for each student in Physical Education.
· T-shirt/Sweatshirt
· Shorts/Sweatpants
· Supportive athletic shoes (no platform or open-back shoes, etc.)
· Sneakers must be laced & tied for maximum support
· Hygiene items such as deodorant (no aerosol cans)
· Optional- towel, plastic bag, soap, brush, etc.
B. Physical Education attire needs to adhere to the school dress code. Clothing that disrupts, or substantially distracts, the educational process will not be allowed.
Examples of prohibited dress: Spaghetti straps, halter-tops, and shirts too short to cover midriff, exposed boxer shorts.
C. Students must change for Physical Education class! (Clothes worn to school that day are not permitted.)
D. No hats, sunglasses, street clothes or sandals are permitted when participating in Physical Education classes.
E. It is recommended that Physical Education clothes be taken home and cleaned when needed.
F. Students will be asked to wear loaner clothes if they forget their clothes. Refusal to dress for Physical Education may result in referral to the opportunity room or after school detention.
G. The following jewelry is not permitted in class:
· Dress watch
· Hoop or dangling earrings
· Necklaces
· Bracelets/Anklets
· Rings
Backpacks are not allowed in class regardless what period you have Physical Education
III.
Time Expectation
· Students are required to be in the locker rooms 3 minutes after dismissal from previous class.
· Students must be in the gymnasium 6 minutes after dismissal from previous class.
· Students are required to participate in the warm-up activity prior to instruction.
· Students are not to leave locker rooms until dismissed by a teacher.
IV. Food
No food, gum, candy, or glass containers of any kind are allowed
in the locker room or any other Physical Education area.
V. Valuables
Any unnecessary items brought to class (i.e. stuffed animals,
toys, pictures, etc.) will be confiscated.
All valuables should be locked in student lockers.
School staff will not be held responsible
for valuables or money lost or stolen in the locker rooms.
We highly recommend that
students keep all valuables and large sums of money at home because they
invite theft.
Please remember to always
lock your lockers.
VI. Locks
A master combination lock will be provided to each student for the
school year. The Physical
Education staff will be the only other people who know the student’s
combination. Students are not
to share their combinations with friends. Sharing locks and lockers may result in lost or stolen items.
VII.
Medical
Excuses- VERY IMPORTANT
A. Students may be excused from class for medical reasons with a written note from a parent or guardian for no more than three days per quarter. You may still be required to participate in class on an adjusted level.
B. Notes written by parents must state the student’s first and last name, the date, the reason for requesting the excuse, the parent’s or guardian’s signature and a phone number where they can be reached.
C. Anything over 3 days may require a doctor’s note.
D. Notes from the school nurse will be handled as follows:
Students must still change for Physical Education class but may not be required to participate, depending on the severity of the illness/injury, as determined by the school nurse.
VIII. Absences/ Tardies
Students who miss or are late to Physical Education class for any reason (including field trips) may be required to make up their work.
VIV. Non-Participation – Extra-Curricular Activities
If a student does not participate in Physical Education he/she is not allowed to participate in his/her after-school activity that day.

Assessment
Students are assessed in Physical Education by using authentic, alternative, performance, or traditional based assessments. They may take any/all of the following forms:

Daily levels

Personal data logs

Class projects (video & computer)

Student Portfolios

Rubric guided skills & projects
·
Tests/Quizzes

Agreement
My child and I have read and discussed the information presented above and understand that it is my child’s responsibility and obligation to follow these Basic Rules and Regulations in order to succeed in Physical Education class. As the parent or guardian, I will encourage my son/daughter to follow the rules and regulations and will support the Physical Education department and school’s action, in the event that my child does not follow the rules. Please fill out this page and return it to your Physical Education teacher. All students are responsible for knowing and abiding by the above requirements.

Videotape/ Photography
Agreement
Occasionally, we may be using videotape and digital photography to provide performance feedback to your child, to display exemplary work on our web site, to decorate bulletin boards, or to help teachers improve a particular lesson. Some student’s projects also require the use of videotape/photos. We ask permission to videotape and/or photograph your child in advance to be able to provide the best service possible to your child and to share the good work we do at Frederick H. Tuttle with other professionals. The videotapes, photographs will not be used for anything other than educational purposes, to maintain student’s integrity and confidentiality. Videos and photographs will not be sold.
Please sign below to allow your child’s image to appear on videotape/photos and our Physical Education web site for educational purposes only. We appreciate your cooperation.
